Thank you for visiting our website, we look forward to the opportunity to be your eye care provider!
Below are a few things to know prior to your visit:

- Please arrive 10 minutes prior to your eye exam to confirm the proper paperwork is completed.
- Please bring any current eyeglasses or contact lenses, as well as a list of any medications or eye drops you are using.
- Please note: if you arrive more than 10 minutes late for your appointment, we may have to reschedule your appointment in consideration of the patients that follow you.

Below are 5 forms for you to print and complete prior to your appointment:
Please note: on your initial visit, you should expect that we will perform refraction and a thorough dilated retinal exam. We recommend that you plan on an appointment length of approximately 90 minutes, but can extend to 2 hours, as it can take 30 minutes for the dilating drops to begin working. In addition, when your pupils are dilated you will be sensitive to light (because more light is getting into your eye) and you may notice difficulty focusing on objects up close. This may last for a few hours in adults and possibly 12 hours in children.
Cancellations
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy on your part, affords the doctors and staff to plan for their day, and allow for possible emergency appointments. A fee may be charged if this policy is not adhered to.
